This form will provide information to the newly forming Health Ministry as to the needs and desires of the congregation and surrounding community to focus on wellness and disease prevention. |

This form can be used to communicate any information for bulletins by 12 noon on Wednesday, and for newsletters the third Sunday of each month only. |

The scholarship is to be awarded to a Reformation member who: (1) is continuing their education after high school, (2) has been active in the life of the church, and (3) who has a demonstrated need. |

By participating in the program, you can use electronic funds transfer to give your weekly, semi-monthly or monthly offering. If you are interested in participating in this program, please print and complete the attached form and bring it to the church office.

We encourage you to use your gifts and talents to The Glory of God at Reformation. There are opportunities to share your gifts; just let us know how you might be willing to help. Trainings are held regularly.
